The ADR CP of SCHOLZ DataBank is the "all in one" representation of Adverse Drug Risks in a Control Panel (ADR). That helps the user to keep the overview over kinetic and dynamic adverse drug reactions especially due to multi drug drug interactions (MDDI).
The new crucial feature is the Quattro Optimizer which enables the user to find quickly alternate drugs when substitution is needed to diminish serious drug risks.
The core of the ADR CP consists of a highly innovative and dynamic literature based drug interaction checker which reconciles the control of MDDI and ADR in a patient medication. Serious MDDI and ADR are easily identified through the alerts signalizing by color and size their importance, meaning severity and probability of occurrence. The alerts are dynamically controlled dependent on how much the plasma level of a drug might be elevated due to either a simple drug drug interaction or a complex MDDI. Therefore, the heart failure risk of dronedarone which amounts with normal dosing to about 10% is assumed to increase up to 100% when concomitant administration of clarithromycin causes a 7-fold increase of the dronedarone plasma level. All patients subject to such scenario would then probably experience heart failure to a more or less severe extent.

A multimorbide patient might need antibiotic treatment with clarithromycin which causes, however, severe MDDI and ADR - as shown in the following screen. Please note in particular the black/red enlarged alerts for the risk of QT prolongation and heart failure as well as the black contraindication alert in the drug listing.

The Quattro Optimizer provides then four tables where the user can simulate and compare what happens when substituting the clarithromycin for example by erythromycin, amoxicillin, or cefaclor.

Please note how especially the risk increases of QT prolongation and heart failure are substantially decreased in the sequence from clarithromycin, erythromycin to cefaclor and amoxicillin, finally to the status before adding any antibiotic. Thereby the ADR for serious adverse effects may be reduced by 90% and the over-all-risk more than halved. The many green highlighted cells also signalize the areas where the ADR is reduced by the substitutions possible.
